No, you do not have to buy the current version. Your old serial number should work for the upgrade from v4.0 to the latest version of Score Writer for Windows.
Where the student writes in the notes? As of now, the rests show and are in the way. I would like to write a staff and put in say, the first note in a scale, and have the students write in the other notes. I would like this without the rests that automatically fill in the measure.

Yes, Score Writer has a "Blank Staff" feature that allows you to write a staff and have students fill in the notes. To access this feature, go to "File" > "New" > "Blank Staff". You can then input any note or rest you want, and students can fill in the rest of the measure with their own notes.

I've checked the software, and it doesn't appear to have a password protection feature. However, since the software is on a shareware license, I believe you're missing the serial number to activate the application. This serial number can be obtained if you verify a customeraccount on the official website through the support section.
The application is still available at the official website. Notice that you will have the possibility to download only a demo version of the program. It must be purchased afterwards if you plan to use it in its full version.
"Brackets" for repeat measures show only endings 1 and 2. How do I add more verses to an ending (in Set Codas, Segnos, etc.) without deleting the brackets?

I recommend using the following forum, because the official website doesn't provide any details or help guides on how to do this. If you have it installed, go to Help > Contents and read about all the available topics related to verses.
I've downloaded and installed the application and then checked that you've said. It Help database contains anything you want related to program's functionality. Start the software, then go to Help > Help Topics and switch to the Search tab. Input text palette then check all the topics. You'll see them listed in the left side of the Help section. Double click one to activate it then read about the palette.
